  • What is a Hazardous Waste Activity Notification Form?
    A Hazardous Waste Activity Notification Form is a legal document that is used to notify the Division of a company's hazardous waste activity or to update information that the Division has on file for a company.

  • When do I need to fill out a Hazardous Waste Activity Notification Form?
    The notification form must be processed prior to conducting hazardous waste activity. You are also required to use this form to notify the Division of any changes to your original notification, i.e. company name, street address of generating site, company ownership, property ownership, type and description of regulated activity and estimated quantity of waste generated.

  • How long do I have to return a completed Hazardous Waste Activity Notification Form?
    Upon the receipt of the notification form, you have 30 days to complete and return the form. If the form is not returned within 30 days your EPA ID number will be rescinded (inactivated) and your company should no longer be generating hazardous waste.

  • If I have not returned a completed Hazardous Waste Activity Notification Form, and the number has been rescinded (inactivated), what will happen if my company generates waste?
    This issue will be referred to the Hazardous Waste Compliance Section for further action, which may include an inspection of your facility.

  • If my EPA ID number is rescinded, how do I reactivate it?
    If your EPA ID number is rescinded, and your company generates hazardous waste, you will need to fill out a Hazardous Waste Notification Form Adobe Acrobat Reader Symboland return it to the RIMS office immediately. You may call (603) 271-2921 to obtain a notification form. You can also download the form from our forms page.

  • What if my company no longer generates hazardous waste?
    If your company does not generate hazardous waste, you do not need an active EPA ID number. To inactivate your ID number, simply fill out a Declassification Request Adobe Acrobat Reader Symbolform and return it to the RIMS's office. You may call (603) 271-2921 and the form will be mailed or faxed to you. You can also download the form from our forms page. However, if you generate hazardous waste in the future, you will need to reactivate your number by completing a Hazardous Waste Activity Notification Form. An EPA ID number may be kept active for a site that does not generate hazardous waste.

  • What if my company no longer occupies a site?
    You need to complete a Declassification Request Adobe Acrobat Reader Symbolform. You may call (603) 271-2921 and a declass form will be mailed or faxed to you or you may download the form through any of the highlighted links. If your company has moved to a new location and is generating hazardous waste, you will need to obtain a new EPA ID number for the new location. *Note: EPA Identification numbers are site specific.
