MUNICIPALITIES that use commercial haulers to handle their solid waste and recyclables shall complete and submit the Towns Without Facilities Report

ALL COMMERCIAL OR MUNICIPAL facilities with a Solid Waste Permit(s) shall complete and submit:
All Facilities Form (Microsoft Word fill-in) with one or more of the following:
- Transfer Stations and Compost Facilities (Microsoft Word fill-in)
- Landfills (Microsoft Word fill-in)
- Incinerators (Microsoft Word fill-in)
The Conversion Table 
will assist you were tons for quantities of waste and recyclables are required to be reported. The table will allow users to change values from cubic yards to tons.
The following steps may be taken if you wish to submit your forms by email:
- Click the above FORM of Interest - a dialog box will appear
- Click SAVE - the form will be saved to your computer
- Open the form through Microsoft Word or the free Microsoft Word Viewer located at the bottom of the page
- FILL out the form
- SAVE a copy of the completed form to your computer.
How to send the form using Microsoft Word
- While the completed form is still opened in Word
- Click FILE
- Click SEND TO
- Select MAIL RECIPIENT (AS ATTACHMENT)
- Compose the email TO donald.maurer@des.nh.gov
- Type in the SUBJECT: "2008 ANNUAL FACILITY REPORTS"
How to send the form using the free Microsoft Word Viewer
- SAVE the completed form to your computer
- Open your EMAIL program
- ATTACH the completed form to the email
- Compose the email TO donald.maurer@des.nh.gov
- Type in the SUBJECT: "2008 ANNUAL FACILITY REPORTS"
By using the above subject will automatically generate a confirmation letter that your form has been received. Without the said subject may delay a confirmation letter.
