*Seasonal Dock Notification for Lakes and Ponds
Notification form used to obtain authorization for first installation of a seasonal dock in a lake or pond for property that meets certain criteria, including shoreline frontage of 75 feet or greater.

Application for projects classified as minimum expedited as defined in Chapters Wt 300 and Wt 500.  Signature by the Town Conservation Commission is a requirement for use of this form. PDF
